Job summary

Jobtailor is seeking a registered nurse for comprehensive home health care in the Bethlehem area. The role involves initial and ongoing patient assessments, admission planning, and coordination with a multidisciplinary team to deliver high-quality home-based care.

Qualifications

Responsibilities

  • Evaluate and perform initial and ongoing comprehensive assessments on patients referred for home health nursing services
  • Determine eligibility for admission, obtain financial and statistical data, formulate initial care plans, and process admissions
  • Refer patients to other agency disciplines and community resources as needed
  • Assess, promote, and maintain the health of individuals, families, and the community through teaching, counseling, and preventive and rehabilitative measures
  • Provide ongoing assessment to determine service frequency, changes in the Plan of Care, and discharge from service
  • Coordinate patient care and assure optimum, comprehensive multidisciplinary care
  • Ensure care plans are followed according to agency standards, quality requirements, service utilization expectations, and third-party payer reimbursement requirements
  • Communicate with physicians, referral sources, patients, and multidisciplinary team members orally and in writing
  • Maintain patient clinical records, reports, and other written communications according to agency policy
  • Record clear goals and plans emphasizing a family-oriented approach to patient care
  • Provide nursing services in patients' homes and places of residence

Job description

Requirements
  • Graduate of an approved school of professional nursing
  • Current professional nurse licensure to practice in Pennsylvania required
  • Current CPR certification
  • Comprehensive knowledge of professional nursing theory and ability to apply it in community health and Hospice settings
  • Ability to instruct professionals in care and treatment of physically and emotionally ill patients
  • Ability to prepare neat, timely, accurate, and concise records and reports
  • Ability to accept and utilize supervision
  • Knowledge of community resources
  • Interest in keeping abreast of trends and developments in nursing practice
  • Two years as a professional nurse in nursing practice or previous community health experience as a professional nurse preferred
  • Good physical and mental health
  • Visual and auditory acuity required to provide comprehensive nursing care
  • Ability to occasionally lift and/or carry 100 lbs
  • Ability to frequently lift and/or carry 50 lbs
  • Ability to stand and/or walk at least 6 hours in an 8-hour workday
  • Frequent stooping and crouching
  • Frequent handling/grasping
  • Occasional fingering/fine manipulation
  • Ability to work in office and patient-home environments
  • Ability to travel to cases and/or clinics in extreme cold and heat, wet/humid conditions, snow, ice, and/or fog
  • Ability to work around contagious disease, road and driving hazards, and domestic pets
  • Part-time and per diem staff must possess a cell phone with text messaging
  • Must use a personal private vehicle for Network business
  • Valid driver's license issued by the applicable state of residency
  • Evidence of auto liability insurance with at least state-minimum coverage limits, maintained throughout employment
