Zusammenfassung

Amae Health in Long Beach, California seeks a Primary Care Clinician (FNP, MD, or DO) for a 1099 contract to deliver holistic primary care to individuals with serious mental illness. The role reports to the Clinical Director and works with a multidisciplinary team.

Onsite commitment is 4–5 hours per week, with responsibilities spanning assessments, medication management, exams, and care coordination integrated with mental health services.

Qualifikationen

• NP Degree from an accredited school or a MD or DO degree from an accredited medical school is required.
• Current, unrestricted license to practice medicine in California.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or related primary care specialty.
• For NP PCP candidates, minimum of 5 years RN experience prior to PCP licensure and 1+ year experience as FNP after completion of clinical hours is preferred.
• Previous experience working independently as a PCP.
• Excellent clinical, communication, and interpersonal skills.

Aufgaben

• Conduct thorough assessments, diagnose medical conditions, and manage acute and chronic illnesses while considering their impact on mental health.
• Prescribe and manage medications, ensuring safe and effective use, while considering potential interactions with psychiatric medications.
• Perform routine physical examinations and preventive health screenings, integrating the findings with mental health care plans.
• Educate members on health promotion, disease prevention, and management of chronic conditions, emphasizing the connection between physical and mental well-being.
• Maintain accurate, up-to-date medical records and documentation that integrate with mental health care plans.
• Participate in ongoing professional development and attend regular team meetings to review and discuss patient care, fostering a collaborative environment.
