Job summary

Amae Health in San Mateo, CA is seeking a Primary Care Clinician (FNP, MD, or DO) for a 1099 contract role. On-site 4-5 hours/week, integrated care for individuals with severe mental illness, reporting to Clinical Director, collaborating with psychiatrists, therapists, peers, and a health coach.

You will conduct holistic primary care, prescribe medications, perform exams, maintain medical records, and participate in team meetings to support coordinated SMI care.

Qualifications

• NP/MD/DO degree required.
• Current unrestricted California medical license.
• Board Certification or Board Eligibility in Family or Internal Medicine.
• Experience working independently as a PCP preferred for NP/PA track.
• Strong communication and interpersonal skills.
• Ability to work in a multidisciplinary team and provide whole-person care.

Responsibilities

• Conduct thorough assessments, diagnose medical conditions, and manage acute and chronic illnesses with attention to mental health.
• Prescribe and manage medications with awareness of psychiatric interactions.
• Perform routine physical exams and preventive screenings integrated with mental health care plans.
• Educate members on health promotion and disease management.
• Maintain accurate medical records integrated with mental health plans.
• Participate in regular team meetings and professional development.
• Uphold compassionate, holistic care and adhere to professional standards.
