Job summary

Blue Ridge Health seeks a Part-Time Patient Navigator to join our Clinical Team in Hendersonville, NC. The navigator will assist patients in accessing care and coordinating services, with a focus on social determinants of health and enrollment support.

The role requires a Bachelor's degree in healthcare or social services (preferred) and the ability to obtain CAC and SHIIP certifications within a month, plus a valid driver's license for outreach and transportation needs.

Qualifications

• Bachelor’s degree in healthcare or social services preferred; equivalent education/experience acceptable.
• Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private insurance, SNAP, and similar programs preferred.
• Ability to become CAC and SHIIP certified within one month of hire.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Screens patients for eligibility and assists with applications for sliding fee, insurance programs, charity care, and other SDH programs.
• Conducts enrollments into programs and provides referrals to partner agencies as appropriate.
• Maintains CAC certification and completes required trainings to stay certified.
• Provides SHIIP counseling to Medicare beneficiaries as requested.
• Educates patients on health center advocacy and voter registration.

What You'll Do:

The Patient Navigator is a member of the clinic interdisciplinary team responsible for facilitating improved health care access and quality for BRH patients through advocacy and care coordination to address social determinants of health (SDH). Responsibilities include:

• Screens patients for the eligibility of and assists with completion of needed applications and paperwork for:
  • sliding fee scale/discount fee program
  • subsidized health insurance programs
  • charity care programs, including local hospital applications
  • medication assistance programs
  • addressing legal issues
  • other programs that assist with social determinants of health (i.e. food insecurity, housing insecurity, transportation insecurity)
• Conducts enrollments into programs when possible, and provides referrals to partner agencies, as appropriate.
• Maintains certification as a Certified Application Counselor for the organization and completes all required training(s) to maintain certification.
• Provides Senior’s Health Insurance Information Program (SHIIP) counseling to Medicare beneficiaries, as requested.
• Educates patients on health center advocacy and voter registration and provides related assistance.

What We're Looking For:

• Bachelor’s degree in healthcare or social services related field, preferred. Equivalent combination of education and experience is acceptable.
• Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.
• Ability to be certified as a Certified Application Counselor (CAC) and Senior’s Health Insurance Information Program (SHIIP) counselor within one month of hire.
• Valid driver’s license and reliable transportation, required
