Job summary

HealthDrive is seeking a compassionate full-time Palliative Care Nurse Practitioner to deliver palliative care to residents in skilled nursing, long-term care, and assisted living facilities. You will partner with residents, families, facility staff, and interdisciplinary teams to relieve symptoms and support goals of care.

This role emphasizes patient-centered care aligned with each resident's values, aims to reduce unnecessary hospitalizations, and improve quality of life.

Qualifications

• Master’s degree in Nursing required.
• Active NP license in state.
• Board certification required.
• DEA/CS license required.
• Valid driver’s license and ability to travel to facilities.

Responsibilities

• Conduct comprehensive palliative care assessments for residents with serious illnesses.
• Manage symptoms and adjust treatment plans.
• Lead goals-of-care conversations with residents and families.
• Coordinate with primary care, hospice, and facility teams.
• Maintain documentation in the EMR and ensure regulatory compliance.
