Brooks Rehabilitation in Callahan, FL is seeking a skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our Outpatient team. This 20hrs/week, part-time role focuses on delivering high-quality patient care in a dynamic, patient-centered environment.

Qualifications

• Current Florida Physical Therapist license or PTA license as applicable.
• CPR/BLS certification required or current.

Responsibilities

• Complete patient evaluations and establish individualized treatment plans.
• Maintain timely and accurate documentation per state and federal requirements.
• Collaborate with the outpatient team to progress through the rehab process.
