Job summary

Brooks Rehabilitation in Jacksonville, FL seeks a skilled Physical Therapist for our Outpatient team. This full-time role (approximately 30 hours/week) focuses on delivering high-quality, patient-centered care in a collaborative, mentor-friendly environment.

You will evaluate patients, develop individualized plans, and work with a multidisciplinary team to achieve optimal outcomes. Florida PT license and CPR/BLS are required.

Qualifications

• Current Physical Therapist license in the state of Florida.
• Hands-on CPR/BLS Certification.
• Knowledge of federal and state professional requirements.

Responsibilities

• Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs.
• Deliver high quality patient care for multiple rehabilitation diagnoses.
• Provide a positive patient experience through patient engagement and progressive treatment.
• Effectively communicate both verbally and written.
