Job summary

Ohana Pacific Health is seeking a passionate Occupational Therapist to deliver home health OT services in Hilo, HI. This part-time role includes developing and implementing progressive rehabilitation plans, serving as case manager, and supervising aides and COTAs to meet patient goals.

Requirements include NBCOT-registered OT, Hawaii license, and BLS CPR. Two years of OT experience, preferably in home health, are preferred. A $1,000 retention bonus is offered along with comprehensive benefits.

Qualifications

• Requires NBCOT-registered Occupational Therapist with current Hawaii license and BLS CPR.
• Minimum 2 years of experience in occupational therapy, preferably home health.
• Familiarity with case management and supervising home health aides/COTAs.

Responsibilities

• Develop and implement progressive home-based rehabilitation plans.
• Serve as case manager and supervise Home Health Aides and COTAs.
• Admit, assess, and discharge patients based on agency criteria and goals.
• Coordinate with interdisciplinary team and community resources.
