Job summary

AED CONSTRUCTORES is seeking a skilled Occupational Therapist for a Skilled Nursing Facility in Sacramento, California. The role focuses on helping patients regain independence in daily living activities after illness, injury, or surgery within a geriatric and post-acute population.

You will develop personalized treatment plans, collaborate with a multidisciplinary team, and monitor progress to improve functional ability and quality of life for our residents.

Qualifications

• Active California OT license.
• BLS/CPR certification.
• Minimum 1 year OT experience, preferably SNF or geriatric.
• Ability to work with a multidisciplinary team.

Responsibilities

• Evaluate and treat patients with physical, cognitive, and functional limitations in a Skilled Nursing Facility setting.
• Develop and implement individualized occupational therapy treatment plans.
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
• Recommend adaptive equipment and environmental modifications to improve independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant documentation of patient care and outcomes.
