Job summary

Concept Rehab & Engage Consulting is seeking a compassionate Occupational Therapist (OTR/L) to join our team in Cedarburg, WI. This part-time role offers flexibility, a supportive therapy team, and meaningful work within a skilled nursing and rehabilitation setting.

You will evaluate patients, develop individualized plans of care, deliver treatments to improve function, independence, safety, and quality of life, supervise OTAs, and participate in care-planning meetings with the interdisciplinary

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist license.
• NBCOT certification.
• Strong clinical, communication, and organizational skills.
• Compassionate and patient-centered approach.
• Skilled nursing/rehabilitation experience a plus.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide skilled OT treatments to improve function, independence, safety, and QoL.
• Establish measurable, functional goals based on patient needs.
• Monitor progress and modify treatment plans as appropriate.
• Supervise and collaborate with Occupational Therapy Assistants.
• Communicate effectively with patients, families, nursing staff, physicians, and the interdisciplinary team.
• Participate in care plan and interdisciplinary team meetings.
• Complete timely evaluations, progress notes, discharge summaries, and documentation.
• Provide education to patients, families, caregivers, and facility staff.
• Recommend adaptive equipment and strategies to improve independence.

Concept Rehab & Engage Consulting is looking for a compassionate and dedicated Occupational Therapist (OTR/L) to join our team Part Time in Cedarburg, WI.

This is a great opportunity for an OT looking for flexibility, a supportive therapy team, and meaningful work with patients in a skilled nursing and rehabilitation setting.

What You’ll Do

• Evaluate patients and develop individualized occupational therapy plans of care
• Provide skilled OT treatments designed to improve function, independence, safety, and quality of life
