Job summary

Concept Rehab is seeking a compassionate Occupational Therapy Assistant (OTA) to join our Cedarburg, WI team on a PRN/as-needed basis. This flexible role supports patients to improve independence and quality of life under licensed OT supervision.

We value a supportive therapy team, ongoing professional growth, and meaningful patient impact. Wisconsin OTA licensure is required or able to obtain. Prior skilled nursing or rehab experience is a plus.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Wisconsin OTA license/certification or ability to obtain.
• Strong communication and interpersonal skills.
• Dependable, compassionate, and patient-focused.

Responsibilities

• Provide occupational therapy treatments under the supervision of a licensed Occupational Therapist.
• Help patients improve strength, mobility, cognition, functional skills, and independence with daily activities.
• Follow individualized plans of care and monitor patient progress.
• Communicate with patients, families, nursing staff, physicians, and other members of the care team.
• Participate in care plan and interdisciplinary team meetings.
• Provide education and training to patients, families, and facility staff.
• Complete accurate and timely clinical documentation.
• Help patients achieve greater independence and quality of life.
