Job summary

Health First is seeking an RN Transitional Care Navigator to perform care management for patients with complex needs. You will assess, develop, implement, and monitor care plans while educating patients to optimize outcomes and reduce readmissions.

This onsite role involves bedside or telephonic patient interactions and cross-team coordination. The position emphasizes collaboration with care transitions teams, documentation, and adherence to clinical guidelines.

Qualifications

• Bachelor of Science in Nursing or equivalent.
• RN licensure in Florida or endorsement.
• Two years of strong clinical practice experience.
• Ability to coordinate care plans and educate patients.
• Familiar with discharge planning and utilization management.

Responsibilities

• Identify high-risk patients and coordinate care across the continuum.
• Coordinate discharge planning to prevent readmissions.
• Educate patients and families to meet health goals.
• Collaborate with physicians, nurses, and care teams for timely transitions.
• Maintain confidential patient information and professional conduct.
