The Pennant Group, Inc. affiliate Symbii Home Health in Afton, WY is hiring a Certified Occupational Therapist Assistant to support care plans under the supervision of a registered Occupational Therapist and ensure quality patient outcomes.

You will evaluate patient status, document progress, and coordinate care with the team while educating families. Licensure, NBCOT eligibility, and CPR certification are required.

Qualificações

• Must be licensed where the HHA is located; licensure required.
• Must have graduated from an ACOTE-accredited OT assistant program.
• Must be eligible for or have taken the NBCOT certification examination.
• A minimum of one year of clinical experience as a certified occupational therapy assistant.
• Demonstrates good verbal and written communication, and organization skills.
• Must have and maintain a valid driver’s license and access to a vehicle.

Responsabilidades

• Participates in ongoing evaluation of patient’s functional status as delegated by the registered occupational therapist.
• Evaluates home environment for hazards or barriers to independence as delegated by the registered OT.
• Teaches new skills or retrains patients in daily activities under supervision.
• Maintains clinical records and reports changes to the registered OT.
• Documents patient responses and progress toward goals.
• Communicates with care team to coordinate and optimize care.
• Attends in-services and case conferences as required.
• Identifies patient/family needs for other home health services and assists with referrals.
• Educates patient-family and other staff in treatment regimes as delegated by the OT.
