Job summary

The Pennant Group is hiring a part-time Certified Occupational Therapist Assistant in Layton, Utah, working under a registered OT to deliver patient-centered home health services. The OTA implements care plans, conducts functional evaluations, and supports independent living for patients in their homes.

Under supervision, you will document patient progress, coordinate with the care team, and help teach patients and families.

Qualifications

• Must be licensed where the HHA is located if licensure is required.
• Graduated from an ACOTE-accredited OT assistant program.
• Must be eligible for or have taken the NBCOT OT assistant certification exam.
• A minimum of one year of clinical experience as a certified occupational therapy assistant.
• Demonstrates good verbal and written communication, and organization skills.
• Must have CPR certification and a valid driver’s license with access to an automobile.

Responsibilities

• Participates in ongoing evaluation of patient’s functional status and needs as delegated by the OT.
• Evaluates home environment for hazards or barriers to independence as delegated.
• Teaches new skills or retraining in daily activities as appropriate.
• Maintains clinical records and reports changes to the OT.
• Follows treatment plans and documents patient progress toward goals.
• Communicates with other care team members to coordinate care and reflect in records.
• Attends in-services and case conferences to ensure coordinated plans of care.
• Identifies needs for other home care services and supports referrals as needed.
• Instructs family/caregivers on treatment regime as delegated by the OT.
