Job summary

Colfax of Cascadia LLC seeks a dedicated Part-Time Occupational Therapist to help residents regain independence and improve quality of life. The role involves patient evaluation, individualized treatment planning, and collaboration with interdisciplinary teams on a rehab program.

You will provide skilled OT services, monitor progress, and adjust plans to meet goals, while promoting safety, dignity, and compassionate care in a long-term care setting. 2–3 days per week.

Qualifications

• Graduate of an accredited occupational therapy program.
• Current state license as an Occupational Therapist.
• Current CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Evaluate referred patients to determine condition, function, and therapy needs.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled OT services to improve daily living skills and independence.
• Assess patient progress and modify treatment plans as needed.
