Colfax of Cascadia LLC is seeking a Full-Time Physical Therapist to help residents achieve independence and mobility. You will develop individualized treatment plans, collaborate with interdisciplinary teams and guide patients through rehabilitation while complying with state and federal regulations.

You will evaluate needs, set goals, implement interventions, monitor progress, and educate patients and families.

Qualificações

• Bachelor's, Master's or Doctorate Degree from an accredited Physical Therapy program.
• Current Physical Therapist license in the state employed.
• Current CPR certification.

Responsabilidades

• Evaluate patients' PT needs and develop individualized plans based on physician orders.
• Establish therapy goals tailored to each patient’s abilities and tolerance.
• Implement, evaluate and modify interventions to maximize outcomes.
• Monitor progress and adjust care plans to achieve measurable goals.
• Educate patients and families on therapy techniques and ongoing care needs.
• Collaborate with physicians, nurses and therapy staff to coordinate care.
• Provide supervision to PTAs and ensure regulatory compliance.
• Maintain accurate clinical documentation in line with regulations.
