Qualifications

• Must hold a valid Occupational Therapy license.
• 1 year of occupational therapy experience preferred.
• CPR certification; valid driver’s license & auto insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform clinical assessment; identifies patient needs and appropriateness of patient.
• Documents, prepares, revises, and completes a Plan of Care, progress notes, and orders.
• Accepts clinical assignments consistent with education and competence.
• Participates in IDG care planning.
• Apply infection control and standard precautions in patient care activities.
• Assess and facilitate ROM, strength, ADLs, and environmental modifications.
• Uses effective interpersonal relations and communication within the team.
• Evaluate patient occupational performance across self-care, work, and leisure.
• Meet continuing education requirements and promote agency policies.
