Job summary

Choice Health at Home in Aurora, CO is seeking a licensed Occupational Therapist to join our patient-centered team. You will assess, plan, and implement therapy programs to improve self-care, mobility, and quality of life for patients across ages.

The role requires a valid OT license, CPR certification, and a driver’s license; HomeCare HomeBase experience is a plus. This full-time position offers a benefits package and a company vehicle subject to availability.

Qualifications

• Valid Occupational Therapy License required.
• 1 year of occupational therapy experience preferred.
• CPR certification required.
• Valid Driver’s License & Auto Insurance required.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform clinical assessment and identify patient needs and appropriateness of care.
• Document and revise Plan of Care, progress notes, and other clinical records in a timely manner.
• Accept assignments within education and competence.
• Participate in the IDG care planning process.
• Apply infection control and standard precautions in patient care activities.
• Assess ROM, strength, coordination, adaptive equipment needs, functional mobility and safety for ADLs.
