Job summary

The Pennant Group, Inc. (Symbii Home Health) is seeking a part-time Physical Therapist in Layton, Utah to join our home health team.

You will assess patients, develop individualized care plans, provide education to patients and families, and collaborate with physicians to ensure a compliant and effective plan of care. Requires a Utah PT license, two years of PT experience (home health preferred), excellent communication and organizational skills, CPR certification, and a valid driver’s license

Qualifications

• Licensed to practice as a physical therapist in Utah.
• Passes state-administered PT exam and holds CAPTE-approved degree.
• Two years of PT experience; home health experience preferred.
• Strong communication and organizational skills; CPR current.

Responsibilities

• Assess and develop an individualized plan of care with patient and caregiver.
• Provide patient, caregiver and family education and counseling.
• Prepare clinical notes and progress summaries tied to goals.
• Coordinate with physicians and health care team on plan of care.
• Deliver PT services per physician plan of care, evaluate function and mobility.
• Instruct patients and families in use of therapeutic devices and exercises.
• Participate in discharge planning and, when needed, staff training.
