Job summary

VNAcare, a not-for-profit leader in hospice care, seeks a Nurse Practitioner to join our Southern California team. The Nurse Practitioner position is open in Rancho Cucamonga and surrounding areas as a Per Diem role during the day shift.

Requirements include a current California Registered Nursing license and a California Nurse Practitioner license, plus a minimum of one year of professional nursing experience. Hospice/palliative care experience is preferred.

Qualifications

• Current California RN license is required.
• Current California Nurse Practitioner license is required.
• Minimum of one year professional Nursing experience.
• Hospice/Palliative Care experience preferred.

Responsibilities

• Provide home visits and patient care within an interdisciplinary team.
• Demonstrate hospice philosophy and nursing judgment.
• Maintain accurate, timely written documentation.
• Work with minimal supervision and self-direct when needed.

The Nurse Practitioner position is open in our Rancho Cucamonga location and surrounding areas. This is a Per Diem position

during our day shift.

Requirements of the Nurse Practitioner:

• Current Registered Nursing license in the State of California.
• Current Nurse Practitioner license in the State of California.
• Minimum of one year professional Nursing experience is required.
• Hospice/Palliative Care experience preferred.

Nurse Practitioner Duties Include:

• Demonstrates an ability to work within an interdisciplinary setting.
• Demonstrates an understanding of Hospice philosophy, principles of death and dying.
• Compliance with accepted professional standards and practices.
• Demonstrate excellent observation, assessment, nursing judgment and communication skills.
• Ability to provide accurate written documentation in a timely manner.
• Self-directed with the ability to work with little supervision.
• Demonstrates ability to efficiently schedule home visits of patients needing a face to face visit for recertification.
