Qualifications

• Comfort working independently in a home-based setting.
• Reliable transportation throughout San Antonio.

Responsibilities

• Conduct home visits for patients recently discharged from hospitals or SNFs.
• Perform medication reconciliation and health assessments.
• Provide short-term management of chronic and acute conditions.
• Educate patients and families on care plans and red-flag symptoms.
• Coordinate care and updates with primary providers and specialists as needed.
• Complete documentation accurately and in a timely manner using EMR.
