Qualifications

• 1+ year of experience in post-acute, geriatric, or SNF care preferred.
• Comfortable practicing independently in skilled nursing facilities.
• Reliable transportation within the Houston area.

Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities.
• Collaborate with facility teams to support quality outcomes and smooth transitions of care.
• Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR.
