Qualifications

• MD or DO with a Texas medical license (unrestricted).
• Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics.
• 1+ year post-acute, geriatric, or SNF care experience preferred.
• Comfortable practicing independently in skilled nursing facilities.

Responsibilities

• Provide in-person medical care to patients in designated post-acute facilities.
• Perform assessments, manage treatment plans, and document in the EMR.
• Collaborate with facility teams to support quality outcomes and transitions of care.
• Engage in Trumen's value-based and transitional care programs where applicable.
• Maintain an on-site presence per schedule and facility needs.
