New Haven in Spanish Fork, Utah is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) for part-time evening shifts. This role supports adolescent girls in a therapeutic setting, emphasizing mental health and trauma-informed care.

You will provide comprehensive nursing care, administer medications, monitor vital signs, coordinate labs and appointments, and communicate with families and the treatment team.

Qualifications

• High school diploma or equivalent; must be at least 21 years of age.
• Active LPN or RN license in the State of Utah.
• Valid Utah driver’s license with a clean driving record.
• Ability to pass Utah State background check; credentialing may require CAQH profile and NPI number.
• Strong communication and teamwork; proficient with medical terminology and charting.

Responsibilities

• Provide comprehensive nursing care for routine needs and medical emergencies.
• Administer and document medications, including controlled substances.
• Conduct timely health assessments and monitor vital signs, weight, mood, and behavior.
• Coordinate lab work and accompany residents to medical appointments.
• Communicate relevant medical information to parents, the Nursing Director, and Treatment Team.
• Collaborate with therapists and psychiatric providers to support treatment plans.
• Participate in an on-call rotation ensuring 24/7 medical support.
• Maintain HIPAA compliance and accurate clinical documentation.

• Mon | 4pm-10pm
• Every other Sat/Sun | 4pm-10pm
