Job summary

New Haven in Spanish Fork, Utah is seeking a licensed practical nurse or registered nurse for a part-time night shift (Mon-Wed 10pm-8am) to provide comprehensive nursing care to adolescent girls in a therapeutic setting.

You will administer medications, monitor vital signs, coordinate labs and appointments, ensure HIPAA compliance, and collaborate with treatment teams to implement individual plans. Must have Utah LPN/RN license, be 21+, and pass state background check.

Qualifications

• Must be at least 21 years old.
• Active LPN or RN license in Utah.
• Valid Utah driver’s license with a clean driving record.
• Able to pass Utah State background check.
• Strong communication skills and ability to work collaboratively in a team.
• Proficiency in medical terminology, charting, and medication administration.
• Skilled in physical health assessments and standard nursing tools.
• Comfortable with word processing and basic network navigation.
• Willingness to receive feedback and participate in ongoing training.
• CAQH profile and NPI number required for credentialing.

• Conduct timely health assessments and monitor vital signs, weight, mood, and behavior
• Coordinate lab work and accompany residents to medical appointments
• Communicate relevant medical information to parents, the Nursing Director, and Treatment Team
• Collaborate with therapists and psychiatric providers to support treatment plans
• Participate in an on‑call rotation ensuring 24/7 medical support
• Maintain HIPAA compliance and accurate clinical documentation
