Compensation is fee for service with an estimated compensation range of $76,068 to $91,282 annually and no cap on productivity income potential.

Qualifications

• Licensed NP or PA with current Delaware license.
• Experience in post-acute care or skilled nursing preferred.
• Strong assessment, care planning, and team collaboration skills.

Responsibilities

• Perform comprehensive patient assessments, histories, and exams.
• Develop and adjust individualized care plans.
• Coordinate care and monitor progress to optimize outcomes.
