Job summary

NPHire is seeking a Locum Tenens Nurse Practitioner to join our Oregon-based program for in-home wellness visits and risk-adjustment assessments. This role supports Medicare members and other at‑risk populations in patient homes across specified Oregon counties.

The position is PRN/Locum Tenens with 32 hours per week, 8-hour shifts, and 4 shifts weekly. Start date is 10/15/2026 for a 52-week assignment with regional travel within Oregon.

Qualifications

• Valid, unencumbered Oregon NP license.
• At least 1 year NP experience; 3 years patient care preferred.
• Experience in primary care, home-based care, or Medicare wellness assessments preferred.

Responsibilities

• Perform annual in-home wellness visits and risk adjustment assessments for Medicare members and other at-risk populations.
• Complete comprehensive examinations including physicals, vital signs, medication review, and functional status.
• Provide patient education on medications, preventive care, and overall wellness.
• Document findings clearly and communicate relevant clinical information to care teams.
• Travel throughout assigned counties and within a 55-mile radius of patient homes.
