Job summary

Impact Health PC is seeking a Part-Time Nurse Practitioner (NP) in Shelton, CT to provide primary care to residents in skilled nursing facilities. You will assess and manage acute and chronic conditions, develop treatment plans, and collaborate with physicians, staff, and care teams.

The role offers flexibility, autonomy, and strong clinical support, with ongoing training and access to physician-level consults. CT licensing and SNF/primary care experience are preferred.

Qualifications

• As a licensed NP, CT license is required to practice.
• 2+ years of clinical experience preferred.
• Experience in SNF or primary care is preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and with a clinical team.

Responsibilities

• Assess, diagnose, and manage acute and chronic medical conditions.
• Develop individualized treatment plans and provide ongoing patient care.
• Complete timely clinical documentation and place orders.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.
• Maintain compliance with facility, state, and federal regulations.
