Sauk County Health Care Center is seeking a Licensed Practical Nurse for part-time NOC shifts (10:30pm–7:00am). The role involves supervising CNAs, delivering geriatric nursing care, and coordinating with the care team to support residents and families.

Qualifications

• Licensed Practical Nurse license is required.
• CPR certification within 6 months of hire is required.
• Geriatric nursing experience is preferred.

Responsibilities

• Prepare and administer prescribed medications to residents and monitor responses.
• Provide comfort to emotionally distressed residents and families.
• Train and supervise CNAs, discuss care plans and evaluations.
• Monitor residents' condition and report changes to the RN during shifts.
• Document nursing tasks and activity in resident records and ECS.
• Accompany physicians on rounds and transcribe orders.
• Collaborate with physical therapists on restorative programs.
• Order and stock pharmaceutical supplies and medications.
• Perform other duties as assigned.
