Job summary

Sauk County Health Care Center is seeking a Licensed Practical Nurse to provide care for residents during the NOC shift (10:30pm–7:00am) on a part-time or full-time basis. The nurse will supervise CNAs, document care, and coordinate with physicians and therapists to ensure quality resident outcomes.

Responsibilities include medication administration, patient comfort, shift handoffs, and maintaining accurate ECS records.

Qualifications

• Licensed Practical Nurse license required.
• Ability to obtain CPR certification within 6 months of hire.
• Preferred: geriatric nursing experience; any combination of education and experience may be considered.

Responsibilities

• Prepare and administer prescribed medication to residents and monitor response.
• Provide comfort to emotionally distressed residents and families.
• Train and supervise CNAs and discuss care plans, evaluations, promotions, and discipline.
• Monitor residents' condition and report changes to RN during shifts.
• Record nursing tasks in residents' records and ECS, including goal summaries and activities.
• Assist physician on rounds and transcribe orders; coordinate with physical therapy for restorative programs.
• Order and stock pharmaceutical supplies.
• Other duties as assigned.

• Order and stock pharmaceutical supplies and medication.

Required Working Hours

Standard working hours may vary between first shift, second shift, and third shift. Please note these hours are subject to change and additional hours may be needed or required. Reimbursement for such additional hours are subject to state and federal regulations.

Minimum Training and Education

Required: Licensed Practical Nurse License
Ability to obtain a CPR Certification within 6 months of hire
Preferred:
Nursing experience with the geriatric population
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
