Qualifications

• Graduate from an accredited Nursing Program.
• Possession of a valid LPN license in the State of New Jersey.
• CPR Certification required.
• Valid Driver’s License is required; must possess own vehicle or transportation to sites.

Responsibilities

• Provide direct patient care under the direction of a Registered Nurse.
• Participate in data collection related to patients’ status and needs.
• Communicate pertinent information to the RN for plan of care.
• Document all aspects of patient care.
• Instruct patients and families on health and care plans.
• Assist in discharge planning and home health maintenance.
