Job summary

University Hospitals in Cleveland seeks a part-time Social Worker to join the Emergency Psychiatric Access Team. The role focuses on psychosocial assessment, discharge planning, and linkage to community resources, working with a multidisciplinary team to support patients and families.

The position alternates between inpatient at Cleveland Medical Center and telemedicine at the Shaker Heights office, totaling 32 hours weekly with night shifts; requires an MSW and LSW license; orientation and

Responsibilities

Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
Provides age specific care to patients/families in all age groups.
Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
Develops plans with attention to individual patient and family goals
Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
Communicates/collaborates with the work team using a multidisciplinary approach.
Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.
