Responsibilities
- Provides psychosocial assessment/intervention to assist patients and families, including clinically complex cases
- Provides age‑specific care to patients and families across all age groups
- Develops, documents, and implements a comprehensive plan based on identified psychosocial and discharge needs
- Develops plans with attention to individual patient and family goals
- Links patients to community resources and UH network services to facilitate optimal care, serving as liaison to community agencies
- Communicates and collaborates with the work team using a multidisciplinary approach
- Offers guidance and acts as a consultant for staff and other disciplines regarding complex psychosocial and/or discharge planning issues and barriers
- Assists with recruitment, orientation, mentoring, and education of new staff and/or social work interns

- Abides by all requirements for protecting and securely maintaining PHI, including annual training and adherence to the UH Code of Conduct and policies
Education
- Master’s Degree from an accredited school of Social Work (Required)
Work Experience
- Hospital or health agency experience (Preferred)
Knowledge, Skills, & Abilities
- Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
- Ability to multi‑task and prioritize work (Required proficiency)
- Works autonomously (Required proficiency)
- Communicates effectively with persons of various backgrounds (Required proficiency)
Licenses and Certifications
- Licensed Social Worker (LSW) – Required
- Licensed Independent Social Worker (LISW) – Preferred within one year of eligibility
Previous social work experience is strongly preferred. Prior work in an emergency room or urgent care is also preferred.
Schedule: Monday‑Friday with rotating weekends and holidays. This position is full‑time, 3rd shift (8:00 pm start time) in the Adult Emergency Department. The schedule can be (3) 12‑hour shifts plus (1) 8‑hour shift every other week, or (4) 10‑hour shifts.
