Job summary

University Hospitals is seeking a Social Worker 2 for the Adult Emergency Room, offering an opportunity to provide psychosocial assessments and interventions. The Social Worker 2 role within the UH Cleveland Medical Center – Adult Emergency Room offers a unique chance to be part of a dynamic, collaborative interdisciplinary team that enhances patient care and assists individuals with a variety of psychosocial needs.

Responsibilities

• Provides psychosocial assessment/intervention to assist patients and families, including clinically complex cases
• Provides age‑specific care to patients and families across all age groups
• Develops, documents, and implements a comprehensive plan based on identified psychosocial and discharge needs
• Develops plans with attention to individual patient and family goals
• Links patients to community resources and UH network services to facilitate optimal care, serving as liaison to community agencies
• Communicates and collaborates with the work team using a multidisciplinary approach
• Offers guidance and acts as a consultant for staff and other disciplines regarding complex psychosocial and/or discharge planning issues and barriers
• Assists with recruitment, orientation, mentoring, and education of new staff and/or social work interns

Education

Licenses and Certifications

Schedule: Monday‑Friday with rotating weekends and holidays. This position is full‑time, 3rd shift (8:00 pm start time) in the Adult Emergency Department. The schedule can be (3) 12‑hour shifts plus (1) 8‑hour shift every other week, or (4) 10‑hour shifts.
