Legacy Hospice seeks a dedicated Master Social Worker (MSW) to deliver compassionate social work services to hospice patients and families. The role emphasizes counseling, emotional support, and helping families navigate end-of-life care.

Schedule: Part Time - 24 Hours per Week, Coverage Area includes Frederick, Clarke, Warren and Shenandoah. Comprehensive benefits, PTO, tuition assistance, and a supportive onboarding program accompany the opportunity.

Qualifications

• Must hold a Master of Social Work (MSW) from an accredited program.
• Licensed social worker preferred; current driver’s license required.
• Minimum 1 year hospice or healthcare social work experience preferred.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Evaluate medical, home, financial resources, and community support needs.
• Develop and implement individualized social work plans with Bereavement Coordinator collaboration.
• Attend interdisciplinary meetings and support care plan coordination.
• Provide empathetic counseling to patients and families.
