Parker Personal Care Homes in Boulder, CO is seeking a Licensed Therapist or Graduate-Level Unlicensed Therapist to provide outpatient, person-centered mental health services to individuals with complex diagnoses in the Boulder/Northern Colorado area.

This part-time clinical role focuses on compassionate assessment, intervention, and case management, delivering services in homes, community settings, or PPCH offices.

Qualifications

• Master’s degree in counseling, social work, or similar field in Human Services.
• Interest in pursuing LCSW, LPC, LMFT, or similar licensure in Colorado.
• Minimum of two years of professional, directly relevant experience is strongly preferred.
• Training or education in trauma-informed therapies is strongly preferred.
• Knowledge of community health care, community-based residential services, and vocational services is preferred.

Responsibilities

• Provide outpatient, person-centered mental health services to adults, adolescents, and children in homes, community settings, or PPCH offices.
• Maintain a caseload of clients, including regular communication with clients and/or caregivers, and scheduling appointments.
• Conduct evaluations, assessments, therapeutic interventions and case management services for clients receiving services.
• Assist with treatment plans, discharge plans, and follow-up care plans.
• Prepares reports and behavioral plans.
• Effectively builds and maintains case files and related documentation.
• Supports access to and participation in the community according to a person’s interests and plans.
• Models respectful and therapeutics support according to a person’s behavioral plan and needs.
• Encourages independent decision-making and self-advocacy.
• Uses internal and external resources for additional services or interventions as needed.
• Complies with State, Federal and PPCH policies, procedures, guidelines and regulations.
