Responsibilities

• Conduct structured interviews to obtain comprehensive medication histories.
• Document medication names, doses, routes, frequencies, and last doses in the EHR.
• Verify and reconcile information from pharmacies and other providers.
• Identify discrepancies and escalate for pharmacist/clinician review.
• Collaborate with pharmacists, nurses, and physicians to ensure accurate admission medication reconciliation.
• Maintain HIPAA compliance and patient confidentiality.
