Holland Lake Nursing & Rehab is seeking an experienced MDS Coordinator (LVN/RN) for a part-time role in Weatherford, TX. You will coordinate resident assessments, ensure regulatory compliance, and monitor documentation and data reporting to support high-quality resident care.

Ideal candidates have 2+ years in long-term care, MDS/PPS and PSSR experience, and fluency in MDS 3.0, with LVN or RN licensure in Texas.

Qualifications

• Must have 2 years of long term care experience.
• MDS / TILE experience required.
• Must be fluent in MDS 3.0.

Responsibilities

• Coordinate Resident Assessments: oversee admissions, annuals, dailys, Medicare and change-of-condition assessments and communicate with nursing and other departments.
• Ensure Regulatory Compliance: follow Federal and State guidelines for MDS assessments.
• Monitor Documentation: review MDS and care plan files for accuracy and organization.
• Utilize MDS software: maintain expertise in software systems for data entry and reporting.
• Lead Interdisciplinary Meetings: coordinate Medicaid, ITTM, and care plan meetings.
• Update Care Plans: adjust care plans based on current assessments and resident status.
• Provide Quality Reporting: generate and distribute Quality Indicators Reports to leadership.
• Submit MDS Data: ensure timely and accurate transmission of MDS assessments.
• Stay Current: attend professional development sessions to keep up with industry standards.
• Oversee Ancillary Data: report ancillary services to corporate billing for processing.
