Job summary

Career Strategies is seeking an experienced MDS Coordinator (RN or LVN) for the South Bay Area. The role focuses on resident care, interdisciplinary collaboration, and strict adherence to state and federal guidelines.

You will complete MDS assessments, ensure timely transmissions, develop individualized care plans, and lead care plan meetings with residents and families, while performing other assigned tasks as needed.

Qualifications

• Current RN or LVN license required.
• Previous MDS experience.
• Flexible with time and able to prioritize effectively.
• Knowledge of AHT and PCC systems.
• Understanding of Medicare ARDs.
• Strong attention to detail.
