ABL Health Care seeks a compassionate Licensed Vocational Nurse (LVN) for home health in Walnut Creek. This part-time per diem role involves assisting in patient care, following the plan of care, and coordinating with a clinical team to ensure high-quality services.

The LVN must hold a California license, have IV therapy and wound care experience, and be able to travel within a 10–15 mile radius. Perks include health, dental, and vision insurance, plus 401(k) options.

Competenze

• CA LVN license required to practice in California.
• IV Therapy certification and wound care experience required.
• Home Health or Hospice care experience preferred (1 year).
• Strong communication and organizational skills.
• Must have a valid CA driver’s license and be able to travel within a 10–15 mile radius.

Mansioni

• Implement the plan of care developed by the Registered Nurse and provide treatment in accordance with patient needs.
• Accurately document patient care in a timely manner, consistent with the plan of care.
• Assess and educate patients and family members/caregivers on diagnosis and care plans.
• Coordinate with other home health services and report needs to the RN or Clinical Supervisor.
• Use equipment and supplies efficiently while adhering to company policies.
• Maintain accurate clinical records and complete online charting in the system.
