Job summary

Omega Home Health Care Services Inc is seeking a Licensed Vocational Nurse (LVN) to provide home health care for our patients across Los Angeles County, Kern County, Santa Maria, Lompoc, San Luis Obispo, and nearby areas. You will work under the supervision of a registered nurse and collaborate with physicians to implement the plan of care.

We offer flexible scheduling with at least two visits per week, weekends one per month, and occasional holidays.

Qualifications

• LVN license is required.
• Active BLS/ACLS/PALS certifications required.
• Proficiency with electronic health records and web-based applications.

Responsibilities

• Provide skilled nursing care per plan of care.
• Teach and promote client health with rehabilitative measures.
• Assist clients in self-care and planning with other health professionals.
• Prepare progress notes and follow care plans.
• Assist in performing specialized procedures as competency allows.
• Prepare equipment and maintain aseptic technique.
• Stay updated on changes in nursing care.
• Attend care conferences and communicate with the nurse.
• communicates patient status to health professionals.
