Job summary

Casa de las Campanas in Rancho Bernardo is seeking a Licensed Vocational Nurse (LVN) to oversee dementia initiatives and provide a therapeutic environment that maintains residents’ physical, social and psychological well-being. The LVN will coordinate with physicians, CNAs and health agencies, maintain medical records, and respond to emergencies with calm, professional leadership.

Qualifications

• LVN with two years of geriatric nursing/assessment experience.
• One year of dementia care experience preferred.
• Strong communication and documentation skills.
• Ability to assess resident needs and respond to emergencies.
• Ability to provide clear direction to Resident Helpers and work as part of a team.
• Compassionate, professional, and resident-focused care.

Responsibilities

• Orders and maintains medications.
• Keeps accurate maintenance of medical records.
• Schedules necessary service for residents on a daily basis.
• Coordinates with physicians and health agencies regarding PT/OT, labs, and other services.
• Assists with scheduling doctors’ appointments and transport as needed.
• Takes action during emergencies and notifies designated personnel.
• Completes and emails shift reports.
• Attends mandatory in-service programs and meetings.
• Participates in facility activities and SCR department meetings.
• Assists in evaluating SCR CNAs.
• Coordinates discharge back to SCR and schedules follow-up care.
