Job summary

Qualifications

• Valid California LVN license is required.
• Six months of LVN experience in residential care preferred.
• CPR/First Aid certification required.
• CPI certification preferred or willingness to obtain.
• Experience with medication administration, G-tube care, injections, and wound care preferred.

Responsibilities

• Administer medications following the five rights.
• Audit medications and verify orders at each cycle.
• Attend medical appointments and relay concerns to the physician and care team.
• Monitor health status and document changes.
• Assess wounds and assist with dressing changes and breathing treatments.
• Perform G-tube care and feeding as needed.
• Administer intramuscular injections when allowed.
• Monitor neurological status after seizures or injuries.
• Observe and report health concerns to the care team.
• Maintain accurate daily documentation and notes.
• Assist with activities of daily living and personal care.
• Provide support during behavioral episodes and ensure safety.
• Participate in community outings as needed.
