Job summary

Redwood Family Care Network is seeking a part-time Licensed Vocational Nurse (LVN) in Antelope, CA. The LVN will provide nursing care, administer medications, monitor health, and advocate for residents with IDD as part of an integrated care team.

Responsibilities include medication administration, wound care, G-tube care, injections, and daily documentation. The role requires California LVN licensure, CPR/First Aid, and CPI preferred.

Qualifications

• High school diploma or GED required.
• Active California LVN license.
• CPR/First Aid certification current.
• CPI certification preferred or willingness to obtain.
• DSP I and DSP II certificates beneficial.

Responsibilities

• Administer medications to residents and follow the five rights of medication administration.
• Audit medications during each cycle and verify orders and rights.
• Attend residents' medical appointments and communicate health concerns to the physician.
• Monitor health status and document changes; report significant concerns.
• Provide G-tube care and feeding as needed within LVN scope of practice.
• Perform intramuscular injections when necessary and permitted by the care plan.
• Maintain daily documentation and client notes.
• Assist with activities of daily living and personal care as appropriate.
• Support individuals during behavioral episodes with safety and dignity.
• Participate in community outings and activities as needed.
