Job summary

Redwood Family Care Network is seeking a licensed vocational nurse (LVN) for a part‑time role in Antelope, CA. The LVN will provide nursing care, administer medications, monitor health, and advocate for residents’ well‑being.

You will work with physicians, DSPs, and care teams, perform basic physicals, document daily notes, manage wound and G‑tube care as needed, and ensure safety and confidentiality in line with the Individual Program Plan.

Qualifications

• Must possess a valid California Licensed Vocational Nurse (LVN) license.
• High school diploma or GED required.
• CPR/First Aid certification required; CPI preferred or willingness to obtain.
• DSP I and DSP II certifications are a plus.
• At least six months of LVN experience in residential care or similar setting preferred.
• Experience with medication administration, G-tube care, injections, wound care, and other nursing procedures preferred.

Responsibilities

• Administer medications following the five rights and verify orders.
• Attend residents’ medical appointments and communicate concerns to the physician and care team.
• Monitor residents’ health status and document significant changes.
• Assess wounds and perform dressing changes and breathing treatments as needed.
• Perform G-tube care and feeding procedures within the LVN scope of practice.
• Monitor consciousness, neurological status, and responsiveness after seizures or trauma.
• Document daily care notes and assist with activities of daily living.
• Coordinate with care team to ensure person-centered care per the Individual Program Plan.
