Job summary

Benchmark Senior Living is seeking a compassionate Licensed Practical Nurse (LPN) for Carriage Green at Milford. The part-time role covers Sunday and Monday, 2:00 PM to 8:00 PM, delivering nursing care in a warm, home-like setting.

Responsibilities include assessing residents, communicating changes to the care team, supervising aides, and coordinating with providers to meet resident needs. Current LPN license and CPR are required; 1–2 years of experience with the elderly is preferred.

Qualifications

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1–2 years of nursing experience; elderly care preferred.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports changes in resident health to leadership per state regulations.
• Directs and supervises daily work of Resident Care Assistants.
• Mentors Lead Resident Care Assistants in job duties.
• Coordinates care with community providers via case management.
• Balances service demands with resident independence.
• Prevents elder abuse and reports per state regulations.
• Responds to emergencies and uses assistive devices as needed.

Carriage Green at Milford is hiring a Part Time LPN for Sunday and Monday, 2p-8p.

We are looking for a compassionate Licensed Practical Nurse (LPN) to join our team! As a Licensed Practical Nurse, your main role will be to deliver nursing care to our residents within a warm, comfortable, and home-like environment. Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations.
• Directs and supervises the daily work assignments of the Resident Care Assistants.
• Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.
• Coordinates care needs with community providers via an effective case management process.
• Effectively balances service demands with supporting resident independence.
• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
• Responds to emergencies and personal emergency response system and knows when to call for backup.

Requirements

• Current state license as an LPN and CPR certification
• Graduate of an approved LPN program (per state requirements)
• Minimum of 1 to 2 years of experience working as a nurse preferred
• Previous experience working with the elderly in a paid or volunteer position is highly desirable
