Job summary

Benchmark at Rye is seeking a compassionate LPN/RN to join our warm, home-like community. You will deliver nursing care and support residents, working with a dedicated team to enhance well-being and quality of life.

Positions include Part Time Days 6a-2p and Full Time Evenings 2p-10p, with hourly pay between $36-$44. Experience with the elderly is highly desirable; CPR and LPN licensure are required.

Qualifications

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program as required by state.

Responsibilities

• Assess, document, and communicate residents’ status.
• Observe changes and report to care team per state regulations.
• Direct and supervise daily tasks of Resident Care Assistants.
• Mentor Lead Resident Care Assistants in job duties.
• Coordinate care with providers via case management.
• Balance service demands with resident independence.
• Report elder abuse/neglect per state rules.
• Respond to emergencies and call for backup when needed.

Benchmark at Rye is looking for a compassionate Licensed Practical Nurse (LPN)/Registered Nurse (RN) to join our team!

Part Time Days 6a-2p

Full Time Evenings 2p-10p

$36-$44/HR

As a Licensed Practical Nurse, your main role will be to deliver nursing care to our residents within a warm, comfortable, and home-like environment. Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations.
• Directs and supervises the daily work assignments of the Resident Care Assistants.
• Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.
• Coordinates care needs with community providers via an effective case management process.
• Effectively balances service demands with supporting resident independence.
• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.
• Responds to emergencies and personal emergency response system and knows when to call for backup.

Requirements

• Current state license as an LPN and CPR certification
• Graduate of an approved LPN program (per state requirements)
• Minimum of 1 to 2 years of experience working as a nurse preferred
• Previous experience working with the elderly in a paid or volunteer position is highly desirable
