Benchmark Senior Living is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care in a warm, home-like environment for our residents in Meriden/Connecticut. You will join a dedicated team and contribute to high-quality care and community support.

The role emphasizes assessing residents, coordinating with care teams, and mentoring aides. A state LPN license, CPR certification, and 1–2 years of experience are preferred.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1–2 years of nursing experience preferred.
• Experience with the elderly in a paid or volunteer role is highly desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports significant changes in resident health to the care team per state regulations.
• Directs and supervises daily work assignments of the Resident Care Assistants.
• Mentors Lead Resident Care Assistants in all aspects of their job.
• Coordinates care needs with community providers via case management process.
• Balances service demands with resident independence.
• Recognizes and reports elder abuse or exploitation per state regulations.
• Responds to emergencies and uses personal emergency response systems.

We are looking for a compassionate Licensed Practical Nurse (LPN) to join our team! Responsibilities

• Accurately assesses, communicates, and documents residents’ status.


• Observes and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations.


• Directs and supervises the daily work assignments of the Resident Care Assistants.


• Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.


• Coordinates care needs with community providers via an effective case management process.


• Effectively balances service demands with supporting resident independence.


• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.


• Responds to emergencies and personal emergency response system and knows when to call for backup.

Requirements

• Current state license as an LPN and CPR certification


• Graduate of an approved LPN program (per state requirements)


• Minimum of 1 to 2 years of experience working as a nurse preferred


• Previous experience working with the elderly in a paid or volunteer position is highly desirable
