Job summary

Qualifications

• Administer medications and injections as ordered.
• Conduct substance use assessments to determine eligibility for treatment.
• Maintain documentation and coordinate care across clinical staff.
• Provide patient education and support to enhance health outcomes.
• Bilingual English/Spanish a plus; experience with Avatar EHR preferred.

Responsibilities

• Administer medication and injections as ordered by the MD.
• Conduct substance use assessments and determine eligibility for treatment.
• Follow up on health-related referrals and coordinate care.
• Maintain patient health tracking and medication records.
• Provide patient education and counseling on health issues.
• Deliver crisis intervention as needed.
• Complete documentation and reports in a timely manner.
• Participate in staff meetings and care conferences.
• Maintain communication with clinical staff to integrate services.
• Ensure compliance with Medicaid and agency policies.
