Job summary

Outreach Development Corp. is hiring a Licensed Practical Nurse (LPN) for a full-time role in Roosevelt, NY. The LPN will provide and coordinate health services under RN supervision, including medication administration and patient education.

The position emphasizes assessment, coordination of care, documentation, and teamwork within an opioid treatment/substance use program environment. Bilingual abilities and experience with AVATAR EHR are a plus.

Qualifications

• LPN license is required and NYS-approved.
• Experience in opioid treatment or substance use programs preferred.
• Bilingual ability is a plus (English/Spanish).

Responsibilities

• Administer medications, including intramuscular injections, as ordered by MD.
• Conduct substance use assessments to determine eligibility for treatment.
• Follow up on health-related referrals and coordinate care.
• Maintain patient health tracking, appointments, and medications.
• Provide patient education and counseling on health issues.
• Provide staff education on health-related topics.
• Respond to medical emergencies with crisis intervention when needed.
• Complete required documentation and reports promptly.
• Participate in staff meetings, case conferences, and unit meetings.
• Communicate with clinical staff to enhance services.
• Conduct toxicology testing as needed.
• Ensure compliance with Medicaid plan and policies.
• Identify health needs of the patient population and address gaps.
