Job summary

Colonial Group in Hudson, MA is seeking an experienced LPN to join our compassionate team. The LPN will provide hands-on nursing care, supervise Resident Care Assistants, and ensure accurate medication administration and documentation in the EHR system.

The ideal candidate holds an Associate’s or Bachelor's degree in Nursing, with a minimum of three years in long-term care and at least one year in a supervisory capacity. Massachusetts RN/LPN license required.

Qualifications

• Associate’s or BSN required.

• Knowledge of medication administration and regulatory procedures; strong organizational and communication skills.
• Licensed Registered Nurse or Licensed Practical Nurse in Massachusetts.

Artisan at Hudson is looking to hire an experienced LPN to join our amazing team!
POSITION SUMMARY: The LPN provides care, a safe environment, and emotional and psychological support to residents. The RN/LPN supervises Resident Care Assistants and ensures that all care demonstrates the We Care values and Sage Associate Promise and complies with state, federal, and corporate guidelines.

• Responsible for care, medication and treatment delivery for residents.
• Supervise the Resident Care Assistants, providing education, evaluations and counseling when needed, ensuring that they are delivering superior care to residents and demonstrating the We Care values and the Sage Associate Promise.
• Responsible for the administration, storage and accountability of medication and treatments.
• Ensure medications and treatments are administered and properly documented. Administer medications and treatments when required.
• Complete all documentation protocols for daily care, emergency care, monthly reports, charting, and medication management. Ensure that all documentation complies with HIPAA regulations.
• Document all care in the Electronic Health Records (EHR) system.
• Report resident concerns or changes to the Health & Wellness Director.
• Ensure Resident Care Assistants are properly documenting care in the EHR system.
• Keep current with state and facility required annual training.
• Perform other duties as assigned.

QUALIFICATIONS

• Education: Associate’s or Bachelor’s Degree in Nursing.
• Experience: Minimum of three years’ experience in personaland/or long term care with at least one year in a supervisory role.
• Skills: Knowledge and competency in medication administration and familiarity with personal care regulations and procedures. Highly committed to resident care and have strong organizational, computer, interpersonal and communication skills.
• Licenses and Certifications: Registered Nurse or Licensed Practical Nurse in Massachusetts.
